Your first visit to Waterstone Dentistry establishes a vital foundation for our relationship with you. During the first visit, we make sure to obtain important background information, like your medical history, and give you time to get to know Dr. Dennis. We encourage questions and do our best to always deliver quality care. Please take a moment prior to your scheduled appointment to download our patient forms. We ask that you complete the forms and bring them with you to your appointment so we may better assist you in a timely manner. If you have any problem getting these files to print properly, please call our office and we will gladly send or fax them to you.

If you have insurance, we are happy to prepare all necessary forms and reports to help you obtain maximum reimbursement from your insurance company.
